The Panic of the System. Another view is that this was a tragic case of groupthink and bad technology. The ships were sailing through a heavy storm with broken radar systems. Jumpy young sailors were listening to sonar in the dark and panicked. Once the false report was sent up the chain of command, nobody wanted to back down or admit they made a mistake. The bureaucracy simply took on a life of its own.
